CHECKOUT - PAYMENT - FEES
After your appointment, you must stop at the check-out
window before exiting. Payment for all services must
be made at the time of your visit unless you have
Medicare. We can print a completed insurance form for
you if you have supplied us with the proper
information. You may mail this form to your insurance
company and they will reimburse you directly. All
services must be paid in full at this time. Credit
cards are welcome.
